Cold River Health uses Elation Passport as the patient portal as of April 2024. If you had a patient portal with our old electronic medical record system, RXNT, that is no longer active. Please contact the office if you need an invitation to be sent to the patient passport.
Unfortunately, Cold River Health's electronic medical record and portal does not integrate with other hospital systems or portals, such as MyChart.

Please fill out the following form to release medical records to Cold River Health. You may also request records from your previous office's department of medical records.

The form can be faxed to 207-747-0402 or emailed to medicalrecords@coldriverhealth.com . Please note, this is not a secure email address and confidentiality cannot be guaranteed if you email the form. There is no charge for releasing records to another medical office. Cold River Health does charge fees set by the State of Maine for release of records other than to medical offices.
